How to Request Holiday Assistance
Holiday food assistance and/or gifts are available to low‐income families, elders, and adults with disabilities in Crook County as follows:

Only one application per household please

Attestation of need and identification required for holiday food assistance

Additional verification of eligibility required for holiday gift assistance

Copy of EBT card with your name or eligibility statement from ODHS, unemployment verification, paystub, or similar documentation

Application deadline extended. After November 15, applications will be accepted only for food assistance and gift requests for children (ages 0-17). Resources may not cover all requests.
Deadline: December 1, 2023, at 5pm.

In 2023, Crook County households may apply for holiday gifts for elders (age 60+), adults with disabilities (age 18-59), and/or children (ages 0-17) if they meet certain income restrictions as follows.
1 person household = maximum gross household income of $1,948
2 person household = maximum gross household income of $2,636
3 person household = maximum gross household income of $3,322
4 person household = maximum gross household income of $4,008
5 person household = maximum gross household income of $4,695
6 person household = maximum gross household income of $5,382
7 person household = maximum gross household income of $6,069
8 person household = maximum gross household income of $6,755
For gift assistance, households need to submit income verification with their application. This can be a copy of your EBT card (with name if you have one) or a copy of your statement of eligibility from ODHS services.

Applications must be returned by December 1, 2023, 5pm, with all information completed to assure processing. Incomplete, duplicate, or late applications will not be processed.
Completion of the application does not guarantee that you will receive the items that you have requested. Early application is encouraged as resources may not cover all requests.
Electronic and paper applications are also available at the NeighborImpact office in Prineville, 457 Ochoco Plaza Drive.

Distribution takes place on December 20-21, 2023. Recipients will receive a postcard in the mail with specific details.
